Social Services: Our social services staff is available to assist with the social concerns and emotional needs of our residents. We will monitor the resident's adjustment and provide services and resources to make them as comfortable as possible during their stay. Social service staff is an advocate for resident's rights are available to assist with all daily concerns of residents and families.
Admission Process: All admissions to Wood Dale must be under the care of a physician and have a current history, physical and orders for medications. Also, a screening and long-term care consultation needs to be done by the Human Services personnel from the county that the potential resident resides in. We admit residents to Wood Dale seven days a week, 24 hours a day. The facility requires copies of Social Security, Medicare and insurance cards, Power of Attorney forms and Health Care Directives. Costs: Nursing home costs are determined by the amount of care that an individual receives. Assessments are completed to determine the level of care of each individual resident. There are currently 36 different rate levels. Our current costs, which are the lowest nursing home costs in the city of Redwood Falls, range from $118.58 to $211.49 per day. In addition, residents are responsible for paying for doctor visits, medications, therapies and phone service. Cable TV reception is provided in each room. Methods of payment include Medicare, Medicaid, Private Long Term Care Insurance, Supplemental Insurances and private payment.
Personal Possessions Needed: It is suggested that resident have at least 5-6 full changes of clothing: sleepwear, tops, pants, underwear, socks and shoes. We do provide incontinent briefs, toiletries, toothbrushes, and combs, Kleenex, etc.. Families are encouraged to personalize a residents room with family pictures, quilts, and mementoes that bring comfort.. You may also bring a favorite chair if room allows and it is safe for the resident. We welcome tours at your convenience.
